Here is a guide on how to write a birthplan that i used when i was expecting it includes almost everything and is very useful.
there is also 1 on cow and gate but i am unsure if i can do a link for that i will leave the answer up to the mods.. If they say yes i will add a link it is a printable version with all the questions rather than a guide
