Have any of you claimed this yet? If so where did you get your form from? I went to my local Job Centre plus today and the woman didn't know what I was talking about. She gave me info about £500 Sure start maternity grant but that wasn't the one I was after, I mean the £190 form that the midwife has to sign.
Any help would be great thanks.

The midwife will issue this form when you see her around the 25week mark, if she doesn't then ask for it as she will sign it when it is issued

I got my form from the midwife too - as they have to sign it.
The criteria is as follows -

You can get the grant if all of the following apply:

you are 25 weeks pregnant or more
your expected date of delivery is on or after 6 April 2009
you have been given health advice from a midwife or doctor

Now the last point causes the problem. My midwives think that is unfair, that just because you have the sense to eat healthy and don't need health advice you do not qualify, so my midwives give it to everyone.

I agree - as this is one of the only non means tested payments I thinkg everyone should get it cause fruit and veg is expensive and I have been buying and eating more and it has cost me a fortune.

Hopefully if you approach your midwife about it they will give you the form.

In fact I got my money already too, it did take almost 4 weeks though just in case anyone else is waiting.

Hi, health advice from the midwife is as simple as the checklist they run down at your booking appointment so as long as you have seen your midwife or GP about your current pregnancy the advice thing isnt an issue!

I phoned my midwife who dropped the form off to me so just waiting on the money now apparently it goes straight into your account no letter or anything.

Yes it does just appear in the account, I noticed that - and then I got a letter 4 days later. Which was not helpful.... Very happy

My money just appeared in the bank and i got the letter over a week later, health advice also applies for being given iron tablets Gag

Thanks ladies, I've got midwife thursday so I'll ask her then. I'm 33 weeks now so I more than qualify, I didn't realise she had them as I'd have presumed she'd have given it to me routinely by now if she has them.

I went on to either www.dwp.gov.uk or www.direct.co.uk put in health start vouchers (assuming thats what you mean??)

I handed in my sure start maternity grant thing to the jobcentre yesterday and she said to me that "we dont take it but i'll pass it on for you".... one thing then if "they" dont take it who does?? Confused Confused

I got the £190 one put into my bank account on thursday there. That was nearly four weeks ago. I hadnt heard so i looked in my bank account on the thursday to see if it had been put in..just aswell i did cos i was gonna phone them yesterday to enquire about it. Glad i checked though or i woulda looked awffy stupid on the phone...
